Nurse Lead - Heart care at home


  • Contract: Permanent, Full-time
  • Hours: 37.5 Hours Per week
  • Working Pattern: Monday – Friday.
  • Work Location: Combination of working from home and office based



Key Roles & Responsibilities:


  • To provide professional nurse leadership to ensure the delivery of safe, high quality, effective and compassionate remote patient care
  • To act as an expert resource, providing advice on nursing and professional issues in relation to the programme, clinical governance, professional standards, policies, protocols and service development
  • To drive transformational change and innovation
  • Work closely with Doctors, Nurse Lead, Remote Monitoring Nurses and operational staff and leaders to design care pathways and care strategies that deliver safe, high quality care for patients included in the programme
  • Promote and maintain good relationship & communication with nursing team and multidisciplinary team
  • Support training & development of nursing team and ensure nursing team working to their max scope
  • Assess work schedules and priorities case for nursing team
  • Provide and efficient, effective, and high-quality service, respecting the needs of each patient
  • Effectively manage time and caseload in order to meet the needs of a changing and developing service
  • Continually monitor the service to ensure it reflects current needs
  • Implement and manage identified changes
  • Ensure that confidentially in relation to patient records is maintained
  • Maintain accurate and up to date statistics of the service provided, including audits & evaluations of nursing services provided
  • Maintain accurate and contemporaneous records / data on matters pertaining to the planning, management, delivery and evaluation of the nursing service in line with Centric Health and Heart Care at home requirements
  • Work closely with researchers on the project to ensure timely and accurate collection and recording of relevant study data, and generation of relevant quality and research reports


Position Requirements:

  • Must be a Registered General Nurse with An Bord Altranais
  • Excellent interpersonal, communication and telephone skills
  • Computer literate
  • Commitment to courteous, sincere and sensitive patient service
  • Commitment to quality and excellent patient care in the practice
  • Excellent organisational skills
  • Flexibility


Preferred but not a requirement:

  • Experience in Heart Failure management
  • MSc level Heart Failure training
