Health Check at Churchtown Medical
Health Check is a proactive approach to your health, aimed at early identification of conditions which could cause illness, reduce your quality of life, and potentially shorten life expectancy.
Patients frequently ask for a check-up and blood tests when they are otherwise well. Blood tests without a clinical assessment are a poor tool for assessing health and can overlook or miss important medical problems. We have designed a comprehensive process to evaluate patients health looking for symptoms and signs of illness and disease. This requires asking questions, carrying out a physical examination, and taking personalised blood and other screening tests. This allows us to give medical advice for current illnesses, and more importantly, reduce risk factors and provide a plan to maintain good health into the future.
Who is This For?
Health Check is available to all adult patients. Patients with a chronic disease and on a medical card may already be registered on the free HSE CDM Programme and receive regular health screening and reviews. However, if you do not qualify for this program the Health Check can be booked online through your practice.
What’s Included in a Health Check?
Your GP and Nurse will provide a detailed clinical assessment and appropriate personalised screening tests. This will include:
- Digital Symptom Checker Questionnaire sent to you after booking your Health Check.
- Two GP and Nurse visits approximately 2 weeks apart. Each visit will take 20-40 minutes.
- Comprehensive top-to-toe examination by your GP
- Blood pressure, Pulse, ECG, Urine screen
- Haematological blood tests including Full Blood Count plus Iron, Vitamin B12 and Folic Acid
- Biochemistry blood tests including Cholesterol, Diabetes, Liver, Kidney, and Bone profiles
- Gender-appropriate blood tests where required including PSA (M) or Hormone Screen (F)
- Additional Blood or Lab tests when indicated following assessment by your GP
- Review with your GP after two weeks to discuss results and a Health Plan
- Patient Information to Support Your Personal Health Plan
How many visits does this Service include?
Your Health Check Journey – Two Visits
- Visit 1: Doctor Consultation (15 min) + Nurse Visit (30 min)
- Visit 2: Return for Doctor Consultation (15 min) two weeks later to discuss results.
