Failure to Attend Policy

Due to increasing demands and several patients failing to attend their appointments, we have decided to develop a policy to reduce the number of unused appointment slots.

If you cannot attend (or no longer require your appointment) please give us 24 hours notice.

You can contact the surgery by phone at 0404 67319 or email info.salem@centrichealth.ie to cancel- we can then offer it to another patient with enough notice.

1st Failure to Attend

You will receive a text informing you of the missed appointment and reminding you to cancel in the future with 24 hours notice, where possible.

2nd Failure to Attend

If you fail to attend for a second time, you will be required to pay a €20 deposit for future appointments. This deposit will be returned upon arrival for your appointment or subtracted from your fee. Failure to attend without cancelling with 24 hours' notice means you will lose your deposit.

3rd Failure to Attend

In the event of a third missed appointment (in 12 months), the Practice will review your case, and consider whether consistent failure to adhere to our Failure To Attend policy constitutes a breakdown between you and Salem Medical Centre. This may affect your ability to book routine appointments in future.
