Continuity of Care: External Investigation Requests

Please note that we are unable to accept or action investigations that have been requested or carried out by another GP practice, including private GP clinics. Similarly, prescriptions and medical certificates must be issued by a clinician within our practice or by a specialist/clinician to whom we have directly referred you. We are unable to process requests for prescriptions or medical certificates originating from external GP services or practices.
