Chronic Disease Prevention Programme at Terenure Medical

A chronic disease is a long-term condition that requires ongoing treatment. Examples include cardiovascular disease (heart-related) and diabetes.

Who is the Programme for?

  • People aged 45 and older, with a medical card, GP visit card, or Health Amendment Act card.
  • Those at risk of heart disease or who have hypertension and those at risk of diabetes.
  • Adults 18+ diagnosed with gestational diabetes or pre-eclampsia since January 1, 2023.

Benefits of the Programme

The programme helps you and your GP work together to reduce your risk of heart disease, diabetes, or both. It includes:

  • An annual review with your GP and nurse.
  • Medicine review and adjustments.
  • Personalised advice on managing your risk factors.
  • Health tips to improve your lifestyle.
  • Necessary medical treatments and referrals to support services, if needed.
  • Care provided close to your home.

How to Register

Contact your GP who will register you for the programme and arrange your annual review.

How the Programme Works

  • It's free. You’ll have one review every 12 months.

     

  • Each review includes a visit with both your GP and nurse (either together or separately).

     

  • You’ll receive lifestyle advice and, if needed, referrals to services like smoking cessation or weight management.

     

  • Blood tests and other necessary tests are free.

     

  • You can still visit your GP outside the programme as usual.

Care Plan

After each review, you’ll receive a written care plan to help manage your risk factors and set health goals with your GP.

Your Information

Your GP will record information such as your name, age, medical history, and test results. This information is securely stored and helps improve understanding and treatment of chronic diseases. You can ask to see your information at any time.

Opting Out

Participation is voluntary, and you can opt-out anytime by informing your GP. You can also rejoin later if you choose.
