Chronic Disease Management: Take Control with Centric Health
Chronic diseases like diabetes, heart disease, and respiratory conditions are on the rise in Ireland, significantly affecting the quality of life for many. If left unmanaged, these conditions can lead to severe complications, impacting both physical and emotional well-being.
Fortunately, the HSE's Chronic Disease Management (CDM) Programme, available at Centric Health GP practices, provides comprehensive care for eligible patients—free of charge.
Why is Managing Chronic Disease Important?
Chronic diseases are long-term conditions that require consistent medical care and lifestyle changes. Factors like an ageing population, poor diet, and inactivity have contributed to the growing prevalence of these illnesses in Ireland.
Properly managing chronic conditions can:
- Improve daily quality of life
- Slow disease progression
- Prevent severe complications like strokes or hospital admissions
With the right support, individuals can lead active, fulfilling lives despite these challenges.
What is the Chronic Disease Management Programme?
The HSE’s Chronic Disease Management Programme offers a structured approach to managing long-term conditions. This GP-led initiative ensures patients receive the right care, at the right time, in the right place.
Here’s how it works:
- GP-Led Care: Your GP oversees your care plan and coordinates regular check-ups with healthcare professionals.
- Personalized Care Plans: Tailored plans ensure your treatment aligns with your unique needs.
- Ongoing Support: Regular monitoring helps keep symptoms under control and prevents complications.
- Self-Management Education: Learn how to manage symptoms, monitor health, and adopt lifestyle changes for better outcomes.
- Access to Specialist Services: Depending on your condition, additional support from dietitians, physiotherapists, or smoking cessation services may be provided.
How This Programme Benefits You
If you or someone you love is living with a chronic disease, the CDM Programme offers significant advantages:
- Early Intervention: Detect and address potential issues before they worsen.
- Better Symptom Control: Regular check-ups and personalized care plans reduce flare-ups and complications.
- Improved Quality of Life: With effective management, you can maintain an active and fulfilling lifestyle.
